Why is condensation forming when I have an extractor fan?

I have an extractor fan set up in my kitchen but condensation is still forming, why is this? How can I fix this issue?

"If the extractor fan is not ducted then you will get the condensation. The Carbon filter reduces the surface area of the air flow hence condensation."
