removing insert and filling

Hi there I have a metal mantel and insert in my fireplace in master bedroom that needs removing, not sure how to go about this.

"Find the holding lugs that are under the plaster unscrew and it will lift out in one . Normally 3 in total above and 2 below"

"A very messy job the mantle will be fixed to your wall in many ways ,metal tangs ,bolts and held in by cement and plaster ,but it will come off ,but messy,messy messy"
